Partnering with the VA
The Homemaker and Home Health Aide (H/HHA) program offers essential support for veterans, allowing them to receive comprehensive in-home care while remaining in a familiar and comfortable environment. This program provides a range of skilled care services tailored to the specific needs of each veteran, ensuring they can continue living safely and independently at home. Services are medically necessary and are prescribed by a VA physician.
To qualify for the H/HHA program, veterans must be enrolled in the VA healthcare system and have a clinical need for in-home care as determined by a VA physician. Eligibility is assessed based on the following criteria:
- Veteran Enrollment: You must be enrolled in the VA healthcare system to participate in the program.
- Clinical Need: A VA physician must determine that skilled care services are necessary.
How to get started with the H/HHA program:
- Enrollment: Ensure you are enrolled in the VA healthcare system. If you’re not already enrolled, visit your local VA office or go online to complete the enrollment process.
- Clinical Assessment: Schedule a consultation with your VA physician to discuss your care needs. Your physician will evaluate your situation and determine if the H/HHA program is the right fit for you.
- Referral: If deemed eligible, your VA physician will refer you to the H/HHA program. This referral will outline the specific services you require.
- Coordination: Once referred, Acti-Kare will work with you and the VA to coordinate the start of your care services. We’ll develop a personalized care plan based on your needs and preferences.
