Acti-Kare Responsive In-Home Care was pleased to participate in the Valley Medical Center Case Management Resource Fair in Renton, coordinated by Retirement Connection.
Deb Packard represented our team and spent the day connecting with case managers, social workers, discharge planners, and other community providers who help patients and families determine what support they may need after leaving the hospital.
These professionals are often helping families make important decisions under significant time pressure. A patient may be medically ready to leave the hospital but still need help managing everyday life safely at home. Family members may suddenly find themselves responsible for meals, personal care, transportation, mobility assistance, medication reminders, and supervision without knowing what resources are available.
